Pharmaceutical Cargo Transport: 2026 Guide to GDP and Cold Chain

Pharmaceutical cargo transport explained: temperature classes, the idea of GDP, documents and permits for medicines and medical equipment moving to or from Iran.

Pharmaceutical cargo transport means moving medicines, vaccines and medical supplies so that they stay inside the temperature and handling limits set by their manufacturer. In practice this rests on three things: the right temperature class, a documented process that follows Good Distribution Practice (GDP), and complete paperwork.

This guide on pharmaceutical cargo transport is written for foreign buyers, agents and importers who send medical goods to Iran or buy them from Iran. It explains the main concepts in plain language. It does not replace the rules of any regulator. Permits and import conditions depend on the product and the country, so always confirm them with the relevant authorities.

Temperature classes in pharmaceutical cargo transport

The product label and the manufacturer's instructions define the storage range. Two ranges are very common in the trade, and airport pharma storage rooms are often built around them. The table below gives a simple orientation.

ClassTypical rangeExamples
Cold chainoften 2–8 °Cvaccines, insulin, many biologics
Controlled room temperatureoften 15–25 °Ctablets, syrups, many devices
Frozenas stated on the labelsome biologics and samples
Ambient, protectedno active controlbandages, simple consumables

Do not guess the class. Ask the manufacturer or the supplier for the exact range, the allowed time outside it and the rules for any excursion. Without this information a forwarder cannot choose the right packaging or vehicle.

What GDP means for pharmaceutical cargo transport

Good Distribution Practice is a quality framework for the storage and distribution of medicines. The European guideline on GDP (2013/C 343/01) has a chapter on transportation. The World Health Organization also publishes model guidance for the storage and transport of time- and temperature-sensitive products. You can find the WHO documents on its distribution guidelines page.

The transport chapter of the EU guideline stresses a few ideas. They apply to pharmaceutical cargo transport well beyond Europe:

  • Use a risk-based approach when you plan the route.
  • Rely on validated thermal packaging, temperature-controlled containers or refrigerated vehicles.
  • Calibrate and maintain temperature monitoring equipment, at least once a year.
  • Keep a written procedure for investigating temperature excursions and unexpected events such as a breakdown.
  • Seal and label the packages, and train drivers on the relevant points.
  • Plan for the maximum transit time, including any storage at customs.

GDP is a quality system, not a single certificate on a truck. Your own quality team or the product owner decides which parts apply to a given shipment.

Road, air or sea: choosing the mode

The best mode for pharmaceutical cargo transport depends on the product, the value, the urgency and the destination. A short comparison helps.

  • Road. Good for neighbouring markets and door-to-door service. A refrigerated trailer holds a set temperature for the whole trip. Our refrigerated transport page describes this service, and the article on refrigerated transport from Iran explains the cold chain in more detail.
  • Air. Fastest option for high-value or short-shelf-life items. Air carriers work with the IATA Temperature Control Regulations and a compulsory time and temperature sensitive label. IATA also runs a CEIV programme for pharmaceutical logistics. See the IATA pharma page for the official description. Our air freight page covers the service, and air freight from Iran covers the routes.
  • Sea. Suitable for large volumes with a long shelf life, often in a refrigerated container. Transit takes longer, so the margin for delay must be larger.

Whatever the mode, ask how handovers are managed. Most temperature problems happen at transfers, at borders and during waiting time.

Documents for pharmaceutical cargo transport

The exact list depends on the product and the countries involved. A typical set for pharmaceutical cargo transport includes the following items.

  • Commercial invoice and packing list with product names, batch numbers and quantities.
  • Transport document: CMR for road, airway bill for air, bill of lading for sea.
  • A shipping note that states the required transport and storage conditions.
  • Product registration or import permission documents, where the importing country requires them.
  • Certificate of analysis or quality documents if the buyer or regulator asks for them.
  • Temperature log or data logger report at delivery, if agreed in the contract.

Names on every document must match exactly. A small difference in a company name or a batch number can stop a consignment at the border. Our customs coordination service page shows how document checks fit into clearance.

Permits and approvals: who decides

Medicines and medical devices are regulated goods, so pharmaceutical cargo transport always starts with permission questions. Import and export normally require permission from the health authorities. On the Iranian side this generally involves the Ministry of Health and Medical Education and its food and drug authority. The importing country has its own regulator too.

We cannot give a ruling on whether a specific product may be shipped. Rules, lists and procedures change. Please confirm permits directly with the competent authorities before booking. Check current trade, banking and insurance restrictions with your bank or legal adviser. Customs documents and the correct tariff code also matter; see our article on the HS code and customs duties.

Medical equipment: more than temperature

Devices and equipment bring other risks. Shock, vibration, humidity and tilt can damage sensitive instruments. Heavy machines need lifting plans and strong packaging. Ask the manufacturer for handling marks, stacking limits and storage conditions.

Good packaging protects equipment during loading and long road legs, which is a core part of pharmaceutical cargo transport for devices. Agree the insured value before departure. If a freight term is unfamiliar, the freight glossary explains it simply.

Frequently asked questions

Is every medicine shipped at 2–8 °C?

No. Many products are stored at controlled room temperature, and some are frozen. The label of the product sets the range, not the forwarder.

Do I need a GDP certificate for the truck?

GDP is a quality approach. Ask the product owner and the regulator what proof they expect. Some buyers ask for validated packaging, calibrated loggers and written procedures.

Can pharmaceutical cargo transport use normal trucks?

Only if the product allows it and the permits are in place. Temperature-sensitive items need controlled conditions and a clear handover plan.

How do I find out the transit time?

Transit time depends on the mode, the border, the season and the documents. A forwarder can estimate it after seeing the route and the product details.
